A Clinical Study of Pilon Fractures

Abstract
An intra-articular fracture of the distal tibia or a pilon fracture is a relative uncommon but . most difficult fracture to manage.
A-elinical analysis- was done on 19 patient-& (21 cases) of the pilon fractures that were admitt.d and treated in Orthopaedic Depai twent, Catholic University Medical College from March 1978 to Feb . 1986. The average duration of the follow up was 15 months:
The summary of the results were as follows:
1. Of the 19 patients, 15 were male, and 4 female.
2. The main cause of injuries was falling from a height, and the other causes were traffic accide it and sports injuries in order.
3. The associated injuries were found in 12 patients (63.1%). The open injuries occured in 8 cases (37..1%). 4. According to the classification of extent of injury by Riledi & Allgower; 4 cases were type I, ¢¥ case type II, and 10 cases type III.
5. According to the clinical assessment of Riledi & Allgower, 7 cases (33.3%) were good, 6 cases (2:.6%) fair, and 8 cases (38.1%) poor. Of the results related to the type of injuries, 3 or 4 cases in type i were good, 4 of 7 cases in type II, and none of 10 cases in type III.
6. Among the method of the treatment, both calcaneal traction and K-wire fixation were not eno igh to get the satisfactory results. Open reduction, screw or plate fixation with bone graft could be ac ieved the satisfactory results.
